To schedule an appointment, just call 414-545-6747 Monday through Thursday between 8:00 a.m. and 5:00 p.m. If you need to cancel an appointment, please let us know so you can avoid missed appointment charges. We’ll send you a courtesy reminder card and call you, so you don’t forget!

We are conveniently located just 1.5 miles south of I-94 in West Allis. Take the Hawley Road exit, which turns into South 60th Street. We are located between Burnham Street and Beloit Road on 60th Street.
